Wormholes in system scan report do not always match the connections shown in the star map.
Occasionally they also do not match the wormholes observable in the nav console.

Examples might help a lot.
Names of the systems or names of cities in the systems.

I've encountered one of these: the starmap shows two surveyed stars connected to an unexplored system, yet neither star's survey reports include the necessary wormhole and orders to travel by wormholes just end up with deadheading. I've sent the system name and coordinates to lightspeed as a diplomatic message since I forgot you need to use the buddies system to send mail.
EDIT: Furthermore I have found a one-way wormhole nearby.
EDIT2: Strangely the destination system has the property of reporting it has more wormholes when you are outside of it. Once inside, the survey agrees with the reality of fewer wormholes. WTH??

We found another solar system with incorrect number of wormholes.
The system map shows 2 negative wormholes, and the star map shows 4 wormhole lines to the solar system.
I made a quick Agents of Hazeron city named Wormhole Research Outpost I.
